Masspro will work with a select group of home health agencies statewide to implement and/or utilize telehealth, including phone monitoring, telemonitoring, or teletriage, to reduce acute care hospitalization (ACH) rates. Acute care hospitalization can be costly to both the patient and payer. The Medicare program estimates that the average cost of an acute care hospitalization is $3,506 per day with an average length of stay of 5.9 days1. Thesefigures would indicate a cost of $20,685 per patient per hospitalization.
Through the use of telehealth to improve ACH rates, Masspro’s goals are also to improve the quality of life for patients and facilitate cost savings. Our clinical, technical, and process improvement expertise will help home health agencies achieve maximum results.
