Care Management
Today, though extremely important in the small-to-medium sized physician practice, care management processes are not well understood and there is little current literature on the process of adoption. Masspro has been engaged by the Centers for Medicare & Medicaid Services (CMS) to develop a framework for the assimilation and dissemination of curriculum, tools and resources to implement Care Management (CM) strategies where the care occurs – the physician practice.
Masspro is working with national and local groups with expertise in the implementation of Care Management, clinical decision support (CDS), patient self-management information technology and process redesign strategies to do the following:

Work with the CDS Community to review, evaluate, and make recommendations on the inclusion of electronic decision supports in the care of patients. Coordinate with other Department of Health and Human Services (HHS) agencies as directed by CMS.

Develop a Technical Advisory Panel (TAP) comprised of experts in care management principles for the small physician office setting. The TAP will make recommendations to CMS as to the content to be developed.

Consider the types and degrees of decision support that best meet the needs of various populations of patients (e.g. the well, the stable chronically ill, the unstable chronically ill, etc.)

Review, analyze, and develop strategies for office practices to become proficient in techniques used to motivate and engage patients’ interest in their own health – so that this information can be shared with physicians’ offices.

Provide and/or develop tools and resources for use in the Care Management process (e.g. care plans, patient contracts) by either/or, as directed by CMS.

Continuously review and assess the environment with respect to the development of new approaches to care management that capitalize on the use of electronic information, and that can be used in the small-to-medium sized physician office.

Develop literature research with information on care management, including references.
